Our In-Network Only (INO) plan is a health plan that offers a large, national network of doctors and hospitals for you to choose from. However, if you choose out-of-network health care providers, it usually will not be covered. In most cases, if you obtain services from a BASS provider, you will have greater savings.

The Bronze PPO plan offers rich coverage with the freedom to choose your own doctor. The plan features in and out-of-network coverage so you and your family can receive the care needed to maintain good health. In most cases, if you obtain services from a BASS provider, you will have greater savings.

The HRA Bronze PPO plan is a PPO plan through Collective Health that we have paired with a Health Reimbursement Account (HRA). The medical plan is the same as the Bronze PPO plan. However, this plan has an HRA, which is an employer-owned account of $1,000 per year which you can use toward qualifying medical expenses under the plan. The HRA is there to help you pay for out-of-pocket expenses as you begin to accumulate toward your deductible.

The HSA plan is a high deductible PPO plan that includes a Health Savings Account (HSA) feature which allows you to set aside pre-tax dollars to pay for eligible health care expenses. In most cases, if you obtain services from a BASS provider, you will have greater savings.

The Kaiser HMO Plan utilizes an integrated managed care approach where most services such as office visits, diagnostic x-ray, lab and pharmacy services can be found at each facility. Under this plan, all of your care must be directed through a Kaiser provider. You will receive benefits only if you use the Kaiser doctors, clinics and hospitals.
